Passport
This page is what is required to be sent to the univeristy.
A valid passport with at least six months of validity remaining beyond the intended stay in the United States.
To write an affidavit of support, there is a general format that we suggest.
There is very limited room to make any changes to the format so please download the letter and ensure your relative can sign it for you.
Use that letter as a format to guide you,
If your University is asking you for a letter from an institute those can be: a school, NGO, any business ie company.
1. While most places ask for 1,500$ per month of support for a year so that would be 1,500*12. This can vary from each state but assume that you would require atleast 33,000$ in personal funds for a year.
2. The big universities require NIH level funding which comes up to 57,000$ per year.
Academic degree(s) and transcript(s): The original academic degree(s) and transcript(s) from the research scholar’s highest level of education.
This is usually your M.B.B.S degree certificate and can also be your provisional certificate issued by your medical school if you do not have one from your Univeristy.
CV or Resume: a Curriculum vitae or resume describing the applicants qualifications, professional and educational background.
It is important to add your permanent address preferably in your home country, this is where you will get your DS form.
You will also need to provide proof of English proficiency.
One way is for your mentor (PI) will write it up for you.
The alternative is asking your home university to mention it in the transcript.
Use the format suggested on the side to help your university draft a letter stating your proficiency in English.
Financial Support Documentation: Financial support documentation, such as bank statements, scholarship letters, or an Affidavit of Support, to show that the research
This is how it should look like, as most likely it is going to be from your personal funds.

Expected timeline.
1. Two to three months to get your DS 2019 form.
2. Five to eight days to help schedule an interview with the embassy. (Any country apart from your home country).
3. Eight to 15 days to get your passport after getting your visa stamped.
Proof of Health Insurance: Evidence of adequate health insurance coverage for the duration of the research.
There are many options you can pick from, the one we would recommend is cheap but it depends on a persons condition.
